Donald W. Black, M.D., is professor emeritus at the University of Iowa Roy J. and Lucille A. Carver College of Medicine in Iowa City, Iowa.

Jordan Cates, M.D., is clinical assistant professor in the Department of Psychiatry and Behavioral Sciences at the University of Oklahoma College of Medicine in Oklahoma City and a staff psychiatrist at the Veterans Administration Medical Center in Oklahoma City, Oklahoma.
